Good luck to you.  I dould not do it after several attempts on-line.  I went to "Chat" and thought I was successful.  They said I would receive it in 3-4 business days.  That was a month ago.  Even my last bill indicated I received it 3 1/2 monthes ago (which I HAD NOT).  I still have not received it and don't expect to receive it.  GOOD LUCK TO YOU.  You might try the phone method.  I would like to hear from anyone that HAS BEEN SUCCESSFUL in receiving one and how they did it.
